Cancellation and Missed Appointment Policy

When we make an appointment for your child, we reserve a certain amount of time to provide you with the highest quality care based on your child’s needs and type of appointment required.  Should you need to cancel or reschedule an appointment, please contact our office as soon as possible, and no later than 24 hours prior to your scheduled appointment.  This will enable another patient who is waiting for an appointment to be scheduled in that appointment slot.

If you do need to cancel or reschedule your appointment, please call our office and we will gladly help you.  If an appointment is missed and has not been canceled or rescheduled, we reserve the right to charge a $50.00 missed appointment fee.

PHA understands that things happen and appointments can sometimes not be canceled in advance.  With that in mind, we will forgive a single missed appointment for existing patients.  However, we may consider dismissal from the practice after 2 missed appointments in a 12 month period.  New families who miss their initial visit will not be given another appointment.

In the event that you are running late for an appointment, please call our office so that we can review the clinician’s schedule and determine if there is still time that day to see your child or if we need to reschedule the appointment for another day.

As a courtesy, we remind our patients of their office visits via email, phone, and text.  Please make sure this information is updated regularly with our staff.
